An exciting opportunity has opened in a local health team. Based at the Zest Centre, the post holder will work within GPs in Upperthorpe, Netherthorpe and Langsett and Winn Gardens to support individuals to self-manage their Long Term Condition and support local communities in leading or changing healthy lifestyles around health and wellbeing.

The post holder must be a pro-active, enthusiastic and highly organised individual who has the ability to engage and motivate people: someone who can deliver one-to-one support to individuals to set their own goals and improve health & wellbeing.

The post holder will work with GPs and local communities to promote positive health and wellbeing support. They will encourage, support and motivate people to develop personal health plans leading to healthy lifestyles and positive long term health improvement.

The post holder will be someone who is organised, self-reliant and IT literate who can plan and organise their own work.

This post offers 25 days holiday rising to 30 (pro-rata), plus 8 statutory days paid holiday (pro rata), a pension scheme and other benefits, including subsidised gym membership

ZEST delivers a range of community based health, employment and training, social and physical activities from the Zest Healthy Living Centre. Zest work with GPs to deliver a health trainer service in the Netherthorpe, Upperthorpe and Crookesmoor areas as well as in Winn Gardens.

Responsible to Line Manager

Purpose of Job

The post-holder will provide support and advice to individuals to develop healthier behaviours and lifestyles in the context of their own local communities.

The post-holder will engage clients working with GP surgeries, offering people practical support to change their behaviour and achieve their personal goals to improve their health and wellbeing.

Responsibilities

  1. To work flexibly as a member of the Zest for Health team, liaising with colleagues as appropriate.

  2. To work with GP surgeries to identify and engage clients who would benefit from health trainer support.

  3. To be fully responsible for a caseload of clients, providing one to one support and advice. You will:

a. Provide accurate information in line with government guidelines about health issues including diabetes, heart health, physical activity and smoking and healthy eating.

b. Support individuals, through motivation, behaviour change methods and empowerment, to help make healthy lifestyle changes and sustain lasting health improvement.

c. Provide advice, setting targets and monitoring clients to realize their personal health plan.

d. Support clients to understand how their behaviour/s might affect their health and wellbeing and understand the barriers they may have in relation to changing this.

e. Signpost clients to access other agencies and services for information, support and resources.

f. Respond in a timely manner to client referrals, in line with the contract.

g. Keep accurate records and enter client data onto the bespoke DCRS database.

h. Report to the Manager the progress of the case load on a regular basis and discuss any arising issues.

  1. Support the promotion of health and health improvement within the local community as appropriate, signposting where appropriate and maintaining good relationships with local agencies.

  2. Take an active part in developing own knowledge and skills.

  3. To carry out any associated administrative duties as required.

  4. To ensure the service works towards best practice in equality of opportunity.

  5. To undertake training and other development activity in line with organisational and personal development needs.

  6. To undertake any other duties as agreed with the Manager.

To carry out all duties and responsibilities as described in line with ZEST policies and procedures, with particular attention to those relating to Equal Opportunities, Health and Safety and regarding volunteers who will work with children and vulnerable adults.

PERSON SPECIFICATION

ESSENTIAL

Skills, Knowledge and Experience

Good standard spoken and written English.

Understanding of health inequalities and their effect on health and wellbeing

Experience of working in a community setting

Awareness of relevant government public health policy and guidelines (e.g. obesity/nutrition/physical activity/smoking/alcohol)

Excellent communication skills and an ability to motivate others

Ability to manage own case load of clients

Strong organisational skills

Ability to work on own initiative

Ability to work in a team

Good IT Skills

Ability to use a database system and carry out data monitoring

Circumstances and Commitment

Able to work evenings and weekends on occasion

DESIRABLE

Experience in a similar role

Experience of working with GP surgeries

Knowledge of the local communities of Upperthorpe/Netherthorpe or Winn Gardens.

Relevant qualifications or training undertaken

Experience of supporting adults in one to one settings

Ability to speak more than one language

Car ownership
