£17,880 per annum + pension + loyalty bonus (funded until July 2008)
Age Concern Sheffield recently launched a new and exciting two-year project (part of the Department of Health funded Partnership for Older People’s Projects – POPPs). It aims to ensure that vulnerable and isolated older people both now and in future years have the services they need to live high quality, independent lives in their own homes.
The successful applicant will be able to work confidently with individuals, groups and organisations. You will have a flexible ‘can do’, problem solving approach, be willing to try new ways of working and be committed to learning and sharing learning. In return you will receive comprehensive training.
The role involves:
understanding the needs and aspirations of the older people living in the neighbourhood in which you will be based, the services available locally and the gaps for future development.
managing a caseload of individual older people, supporting them to access the services they want and need.
building links between voluntary, community and faith organisations and groups, health and social care professionals and others who come into contact with older people.
